Q. Can I change how results are sent to me?
A. Yes. If you need to change or update your contact information for releasing results (or to request an additional copy of the results) you must sign and date a completed Report Release Form.

You must also provide a copy of your photo ID. Once we receive your written request and authorization, we can send results to the new address. Additional hard copy reports are $25 each.
